Position Title
Director of Behavioral Health Integration
Direct Supervisor Managing Director of Social Services
Position Summary Responsible for oversight of integrated behavioral health care programs including MAT, projects and staff across the organization. Participates in the successful development and implementation of Health and Behavioral Health Integration (BHI) and Quality Improvement initiatives, both internally and with stakeholders and community partners. Facilitating a variety of direct care and administrative services in an outpatient setting. Implementation of best practices, trainings and workflow development. Development and implementation of policies and procedures aligned with grantor, City, State and federal guidelines. This includes preparing reports, grant, tools, metrics and best practices that positions CCHC to access funding sources for health services in light of healthcare reform, especially as this pertains to BH and SUD including Medication Assisted Treatment services. Infrastructure & capacity development, systems strategy & implementation, and behavioral health business strategy are within the scope of this position.
Minimum Qualifications
Master’s degree in Social Work or other healthcare-related field. Organized healthcare facility experience and group psychotherapy training/certification strongly preferred.
A minimum of five years of progressively responsible related leadership experience that would demonstrate attainment of the requisite job knowledge/abilities.
PsyD, and or current LCSW or LCPC licensure, CADC or SUD care experience required with Program development, grant administration, policy development and implementation. Telehealth experience a plus.
Understanding of the current behavioral health system and health payment structures. Experience with and understanding of primary care/behavioral health integration and nonprofit management.
Knowledge of complex medical and mental health conditions and national standards of quality care.
Capability to work collaboratively with people of diverse cultural and religious backgrounds
Strong analytical, problem-solving and big-picture/conceptual skills
Capacity to embrace and understand the change process and effectively support change management.
Excellent oral and written communication, planning and decision-making skills.
Experience managing or supporting cross organizational partnerships and collaborations
Demonstrated strong verbal/interpersonal and written communication skills
Ability to exercise sound judgment and to make timely, value-based decisions
Ability to excel in fast-paced, changing, and challenging environments
Fluent in Microsoft Office applications, including Word, Excel and PowerPoint, etc. Educational & Experience:
Responsibilities
Provide direct clinical care at least 10 hours per week using best practice standards and screening tools. Helping participants in setting individualized goals for themselves and in assessing their own skills and resources related to goal attainment. Goals are set by exploring strengths and needs in the individual’s living, learning, social, and working environments. Implementation of services in the plan may take place individually or in groups.
Consistently share best practices across agency and networks, and standardized information and resources
Works collaboratively with CCHC leadership and strategy leads to support the enterprise in achieving strategic planning and quality strategy goals.
As a subject matter expert, the Director supports efforts in business development including the design and implementation of various BHI programs on behalf of CCHC
Supports implementation of behavioral health integration programs across CCHC. Successful implementation includes the proposal of standardized processes, quality and performance measurement.
Works by incorporating Population Health strategies, with clinical directors, providers, care and case management teams and community health workers in quality and best practice standards to ensure BH is integrated into care management and practice support functions.
In collaboration with CMO, develops training materials and implementation plans for working with populations around best practices in management of populations with BH and chronic medical needs, information around BH conditions, BH resources, etc.
Provides technical support in an effort to maximize collaboration with health systems, primary and specialty care practices, healthcare payers and other health, behavioral health and nonprofit sectors.
Aligns the BHI program goals with the priority goals of CCHC
Assists in planning and facilitating webinars and onsite trainings and attending external collaboration meetings as required.
Monitor and evaluate team schedules, and productivity templates
Maintain and submit weekly/monthly statistical reports related to program services performed (service referrals received, internal and external depression screens, domestic violence screens, individual counseling sessions conducted, etc.)
Document all client encounters and contacts made on behalf of clients utilizing prescribed forms; develop, complete and submit weekly progress reports.
Assist in an on-going appraisal of the program, which will result in continuous improvement in the quality of the program and in the development of new services.
Participate in multi-disciplinary staffing program and clinical meetings.
Perform other duties as assigned
