Social Services Director
to lead our department and support the emotional and social well‑being of our residents. The Social Services Director plays a vital role in resident adjustment, care planning, and discharge coordination, ensuring compliance with all state and federal regulations.
Social Services Director Responsibilities
Oversee daily social services operations, including staff supervision and scheduling
Assess resident needs and develop individualized care plans
Support residents and families through admission, transition, and discharge processes
Ensure compliance with documentation and regulatory requirements
Coordinate discharge planning and community resource referrals
Participate in care plan meetings and quality improvement initiatives
Promote a culture of compassion, teamwork, and resident‑centered care
Social Services Qualifications
Bachelor’s degree in Social Work or related field (Master’s preferred)
3–5 years of social services experience; 2+ years in leadership preferred
Experience in long‑term care or healthcare required
Knowledge of regulatory standards and documentation practices
Strong communication, leadership, and organizational skills
