What You’ll Do as Social Services Director
Complete
pre-admission screenings , including mental health evaluations per facility criteria
Conduct and update
comprehensive social services assessments
Serve as
Care Plan Coordinator
as assigned
Collaborate with the interdisciplinary team to develop and evaluate individualized care plans
Protect and promote resident rights and psychosocial well-being
Document resident progress notes, observations, and interventions
Manage vendors, specialists, and supportive services visiting the facility
Support residents and families through admissions, discharges, and transitions of care
Perform additional duties as needed to support overall resident care and facility operations
What We’re Looking For
Bachelor’s degree required
Minimum 2 years of experience
in long-term care or a related healthcare setting
Strong knowledge of
state, federal, and Department of Health regulations
Experience with the survey process highly preferred
Excellent communication and teamwork skills
Compassionate, resident-focused, and dedicated to quality care
