Overview

MedStar Health is looking for a Peer Recovery Specialist to join our team in Rosedale, MD. The Peer Recovery Specialist applies their personal, lived experiences to the non-clinical work of coordinating follow-up care, providing linkages to community resources, and sharing their experience, strength, and hope to patients in need. They will work with the Care Management/Social Needs Team at MedStar Health's Care Transformation Organization (CTO). Services are provided in the office, patient homes, and community settings. Primary Duties

Act as peer support throughout the patient\'s course of care with the Care Transformation Organization. Act as a resource to other clinical team members on recovery support. Assist patients and families with understanding how to utilize community services. Assist patients in linking to community support services and substance treatment services including securing appointments and transportation. Complete required documentation and other reports. Conduct brief interventions for patients that screen positive for alcohol and/or drug use. Identify patient\'s history of mental illness and/or drug/alcohol use, treatment history, motivation to change behavior, and other needs for community-based supports. Identify patient\'s readiness to change and use motivational interviewing techniques and other communication skills to support a plan for reducing or eliminating harmful drug and/or alcohol use. Model coping techniques and self-help strategies. Participate in training to continually gain new skills. Provide patient\'s family members with recovery support materials. Provide community-based in-person outreach and services to patients to assist with linkages and continued peer support. Telephone contact to verify patients showed up for an appointment; follow-up by telephone with any patient that misses an appointment. Work with CTO staff to identify community resources. Consult with care management and social needs staff. Qualifications

High School Diploma or GED. Must be actively engaged in own recovery program with at least three years of personal recovery and sustained abstinence from alcohol and/or drug use with no relapses. Certified Peer Recovery Specialist (CPRS) attained within 6 months of hire is preferred.
