As a hospice liaison, your role will involve: Coordinating patient referrals and establishing vital connections with local hospitals to secure these referrals. Educating the community and healthcare professionals about the benefits and services offered by hospice care. Working closely with patients and their care teams to guarantee seamless continuity of care, effective communication, and precise documentation. Essential Functions: The Hospice Liaison will focus on coordinating and generating hospice business while identifying and maintaining relationships with existing and potential accounts. This role serves as the critical link between our agency, hospitals, and other referral sources, educating the community about our valuable hospice services. Your interactions each day with patients, medical professionals, and the community will ensure that care is delivered effectively and efficiently. Qualifications: A genuine passion for serving patients and families and a commitment to providing exceptional care. Education:

Minimum of an Associate Degree or proven experience in a health-related field. Experience:

A minimum of 2 years of sales experience in a clinical care setting, with hospice experience preferred. Requirements:

Reliable transportation and the ability to perform physical tasks related to the job, including sitting, standing, bending, moving intermittently, and lifting at least 25 lbs.
