R-264**Job Description:**Qualifications:* Must successfully complete the required pre-employment Background and Drug Screenings* Must possess a valid Driver's License which meets company standards for approved driver status* Must possess basic computer skills* Must possess a Bachelor’s Degree in the Human Services field* Must have at least one year experience working in the field of intellectual/developmental disabilities* Experience in writing program plans, case notes or other relevant documentation preferredEssential Functions:* Oversees and coordinates the Program Plan of each individual and ensures it aligns with the individual's needs and desires* Creates positive, collaborative communication with all disciplines and team members* Ensures that Program Plans and documentation are completed in accordance with all regulatory agencies* Assists in monitoring and training of staff and relevant parties to ensure accurate, implementation and documentation of training, support and behavior programs as required by the individuals’ plans* Responsible for assisting individuals to apply for third party benefits such as Medicaid and Medicare* Performs all other duties as assignedThis position will remain open until filled.
