JOB SUMMARY: The Regional Social Services Director provides leadership, guidance, and education to Social Services team members across multiple skilled nursing facilities. This role is focused on practical support, regulatory compliance, and problem-solving, not clinical therapy or psychological services.
The Regional Social Services Director partners closely with facility Social Services staff, Administrators, DONs, and interdisciplinary teams to strengthen social services practices, support staff development, and ensure consistent, high-quality resident and family support across all communities.
RESPONSIBILITIES:
Support onboarding and training of new Social Services team members.
Provide ongoing guidance, mentorship, and education to strengthen confidence and consistency.
Assist facilities during transitions, vacancies, or increased support needs.
Ensure Social Services practices align with state and federal regulations (CMS, survey requirements).
Support survey preparation, audits, and follow-up action plans related to psychosocial services.
Review documentation for accuracy, compliance, and best practice standards.
Assist facilities in resolving survey citations related to Social Services.
Support facilities with complex resident or family situations.
Assist with discharge planning challenges, care conferences, grievances, and family concerns.
Help teams navigate guardianship, placement, behavioral concerns, and community resources.
Provide practical solutions and direction to move cases forward efficiently.
Develop and deliver education, tools, and resources for Social Services staff.
Standardize workflows, documentation practices, and communication processes.
Identify trends or gaps and recommend improvements across facilities.

REQUIREMENTS:
Master's Degree in Social Services or similar field.
5+ years of Skilled Nursing and/or Long-term care experiencerequired.
Bachelor’s degree in social work, Human Services, Psychology, Sociology, or a related field.
Experience working in skilled nursing or long-term care social services.
Strong understanding of discharge planning, resident rights, and psychosocial regulations.
Ability to travel between facilities as needed.
Practical, solution-oriented mindset with strong follow-through.
