Responsibilities:
Identifies cognitive impairments, mood problems and other psycho-social needs in residents. Maintain accurate, thorough, and timely documentation. Review Advanced Directives and provide education to residents and family members as needed. Help provide a smooth admission and discharge process for residents and family members. Other duties as assigned by Administrator. Qualifications:
Ability to work independently or part of a group. Computer knowledge. Ability to build and maintain rapport with residents and peers. Prior Long Term Care/Skilled Nursing Experience in Social Services or Required LSW CSW Certificate required Complete Care at Hagerstown is an equal opportunity employer.
