Job Title: Community Health Worker
Location: 201 LYONS AVENUE, NEWARK, NJ 07112-2027
Facility: RWJBarnabas Health Corporate Services
Department: RWJBH Community Health Project
Pay Range: $21.69 – $30.64 per hour (hourly)
Status: Full‑Time (Hourly), Day shift
Job Overview The Outreach Community Health Worker will primarily maintain a presence in the community with patients to promote, motivate, and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.
Qualifications Required:
High School Diploma or equivalent
Essential Functions
Meet patients in a clinic, hospital, ER, and where patients are located and conduct needs assessment using Social Determinants of Health Tool
Schedule weekly follow‑up calls and home visits to patients
Help patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members' lives
Assist patients with organizing their records, making follow‑up appointments with a Primary Care Physician or Specialist
Help patients with the tasks of getting medical care, and work on health goals by helping to plan healthy meals or finding time to exercise
Help patients access resources, including quality of care, childcare services, food, housing, transportation
Maintain relationship with the Community Based Organization
Able to travel and meet the members where they are; may include local churches, homeless shelters, and members' residence
