As an Enrollment Specialist, you will be an integral part of our Patient Engagement Specialist (PES) team, engaging with patients over the phone to educate them and enroll them in our Medicare-sponsored care coordination program. This program is designed to assist patients in effectively managing chronic conditions such as diabetes, high blood pressure, and COPD.
