Roles & Responsibilities

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist to help residents regain strength and mobility
• Assist residents with therapeutic exercises and mobility training, promoting independence and safety
• Monitor patient progress and report meaningful updates to the supervising PT to inform care plans
• Maintain accurate, timely, and professional documentation to support quality care and compliance
• Collaborate with the interdisciplinary healthcare team to coordinate patient-centered treatment goals
• Communicate clearly and compassionately with residents and families to encourage participation and progress

Qualifications

• Active Ohio PTA License (required)
• Current BLS (Basic Life Support) certification
• SNF experience preferred — experience in skilled nursing is a plus
• Strong clinical judgment and hands-on patient care skills
• Excellent communication and teamwork abilities
• Willingness to work PRN/per diem shifts and adapt to facility scheduling needs

Knowledge & Skills

• Skilled in therapeutic exercise prescription and mobility training
• Proficient with patient assessment and progress monitoring
• Strong documentation practices and attention to detail
• Ability to collaborate effectively with nurses, therapists, and care staff
• Knowledge of safety and fall-prevention strategies in elder care
• Compassionate patient-centered care and strong interpersonal skills
