A Home Health Physical Therapist (PT) provides one-on-one, skilled rehabilitation services in patients' homes to restore mobility, improve function, and increase independence. Key duties include conducting initial evaluations, developing individualized plans of care, implementing therapeutic exercises, and educating patients/caregivers on safety and fall prevention. They frequently treat geriatric patients, post-surgical cases, and those with chronic conditions, often acting as case managers. Core Responsibilities & Duties Assessment & Care Planning: Evaluates patient physical, functional, and home safety needs to create, implement, and revise individualized plans of care (POC). Treatment Delivery: Administers skilled therapeutic techniques, including manual therapy, therapeutic exercises, gait training, and neuromuscular re-education, within the patient’s home environment. Patient/Caregiver Education: Instructs patients and families on proper use of assistive devices (walkers, canes), home exercise programs, and pain management strategies. Documentation & Reporting: Maintains detailed, timely electronic medical records (EMR) regarding patient progress, goals, and changes in condition. Collaboration: Communicates with physicians, case managers, and interdisciplinary team members (nurses, PTAs) to coordinate care. Safety & Compliance: Ensures a safe, clean treatment environment, adheres to infection control protocols, and complies with Medicare/insurance regulations. Key Qualifications & Requirements Education: A Doctor of Physical Therapy (DPT) or equivalent from an accredited program. Licensure: Active, current state licensure as a Physical Therapist. Certifications: CPR certification is required. Experience: Previous experience in home health, acute care, or rehabilitation is frequently preferred. Skills: Strong time management, organizational, and interpersonal skills. Must be familiar with Oasis documentation for Home Health Transportation: Valid driver’s license and reliable transportation to travel between patient homes. Working Conditions Environment: In-home, which requires adaptability to various environments, including homes with pets, smokers, or limited accessibility. Autonomy: High degree of independence in scheduling and patient care. Physical Demands: Requires walking, standing, bending, lifting, and assisting patients with mobility.
