Position Summary:

We are currently hiring a

Bilingual (Spanish/English) Facilitated Enroller

to join our Outreach & Enrollment team at Urban Health Plan. The Facilitated Enroller provides community outreach and assists individuals in successfully applying for and enrolling in Health Insurance programs that fit their needs. As part of the FE-ABD program, the FE emphasized services for community members whom are aged, blind, disabled.

The ideal candidate must be able to work independently, conduct outreach within the community, educate patients about available health coverage options, and effectively manage multiple tasks in a fast-paced environment to support patient care needs.

Minimum Qualifications: High School Diploma required. Minimum of 1-2 years of experience with clerical/administrative functions is required, ideally in healthcare or support services setting. Bilingual Spanish/English is required. Must have excellent communication skills and ability to provide exceptional experience to patients. Understanding of phone etiquette and standard office procedures. Ability to thrive in a fast-paced environment. Computer Proficiency required. Key Duties:

Secure and maintain training credentials as a Facilitated Enroller. Coordinate and collaborate with the Facilitated Enrollment of local and statewide teams. Facilitate ongoing monthly community outreach that aims to educate agencies and community members about the Facilitated Enrollment Program. Facilitate phone outreach to individuals who qualify for Medicare and who may wish to receive assistance with the Medicare program. Professionally handle incoming inquiries and referrals by providing information about the Facilitated Enrollment Program and other relevant services. Coordinate with and assist individuals to facilitate their complete application and enrollment process. This process can be accomplished in-person, by phone or via virtual meetings. Track and maintain records of completed outreaches and applications to ensure achievement of monthly quotas and reporting deadlines. Ensure timely and accurate reporting of program data for both specific program requirements as well as State and Federal reporting requirements. Submission of complete and accurate applications. Other related administrative/Outreach duties as assigned. Starting Compensation:

Minimum $20.67 per hour; Maximum $20.67 per hour based on full time, 40 regular hours per week.

Work Environment Type, Location, and Schedule:

Applicants must be comfortable working onsite and working across different locations at Urban Health Plan when required (including Bronx, Manhattan, and Queens Clinics). Schedule requires- Monday to Friday 8:00 am to 5:00 pm and Occasional Saturday (for events).
