Client: Established Home Care Organization

Role: Home Care Marketer

Pay Rate: $60,000 – $75,000 base salary + performance-based incentives

Location: Ohio, Cleveland/ Akron (Northeast Ohio Territory)

Position Overview The Home Care Marketer will be responsible for driving census growth and expanding referral partnerships within the local healthcare community. This role focuses on building strong relationships with hospitals, physicians, rehabilitation centers, senior living communities, and case managers to increase referrals and brand visibility. The ideal candidate is relationship-driven, results-oriented, and experienced in healthcare outreach.

Key Responsibilities

Develop and execute territory marketing plans to drive patient referrals and admissions

Establish and maintain strategic partnerships with hospitals, discharge planners, physicians, rehabilitation facilities, and senior living communities

Conduct in-person outreach visits, presentations, and networking meetings within the assigned territory

Educate referral sources on services, care programs, and differentiators

Track referral activity, conversion metrics, and market trends

Collaborate closely with clinical and intake teams to ensure seamless transitions and superior onboarding experiences for new patients

Represent the organization at community events and healthcare networking functions

Maintain CRM records and provide regular pipeline and performance reports to leadership

Qualifications

Bachelor’s degree in Marketing, Business, Healthcare Administration, or related field (preferred)

2–5+ years of healthcare marketing, home care marketing, or business development experience

Established relationships within the local healthcare community strongly preferred

Strong understanding of Medicare/Medicaid and home care services

Excellent communication, presentation, and relationship-building skills

Self-motivated with the ability to work independently in a field-based role
