Overview

Under the supervision of the Clinical Supervisor, the Housing/Case Manager will provide case management services to a caseload and provide housing assistance within the facility. Compensation Min USD $46,000.00/Yr.

Max USD $51,500.00/Yr. Key Essential Functions

Case Management: Provide case management services to a caseload of up to 15 Manage and develop systems in the facility Complete all paperwork for charting as required by funding source Conduct groups periodically or as directed by management Housing Services: Provide Housing Assistance to clients Complete Housing Assessments and Referrals Complete HRA 2010-E’s (CAPS) ADDITIONAL FUNCTIONS MAY INCLUDE: Attend management trainings, team meetings, and staff meetings Provide assistance within the facility where needed Performs other duties as required by the Clinical Supervisor Qualifications

Educational / Training Required: Either CASAC or Bachelor degree Preferred GED/High School Diploma or Associates Degree Required CASAC-T required Experience Required / Language Preference: Minimum 2 years’ experience with population in a residential setting or related field Computer Skills Required: Proficient in Microsoft Office and related programs Company Overview
