Pay Range: $30-$34 DOE
Responsibilities
Inform new residents of their personal and property rights and provide assistance to residents and families to ensure a satisfactory adjustment to life in the Center
Address difficulties with emotional adjustment
Coordinate behavior management programs and provide behavioral and emotional support to residents as needed
Ensure the timely completion of social services evaluations, progress notes, and MDS tasks
Manage and audit residents' advance directives, PASRRs, psychotropic medications, and care conferences
Assess and implement social services care plans for all residents
Facilitate resident discharges and coordinate all discharge needs, such as Home Health, DME, DSHS, etc.
Monitor and collaborate with outside agencies to ensure quality interventions for residents needs
Serve as primary resident/patient/family liaison regarding communication and development of IDT treatment and/or discharge plan
Develop a Family Council and coordinates meetings
Supervise/oversee Social Services Assistant
Follow OSHA and Infection Control standards
Adhere to all CMS and State regulation pertaining to social services
Additional Duties
Participate in training programs
Interdisciplinary tasks as assigned
Assist with special center programs and events
Handle admission inquiries and Center tours as needed
Requirements
Minimum 1 year of experience in a long-term care setting
Bachelor's Degree in Social Work or Human Services field
