Position Overview The Director of Population Health at our Wilson Community Health Center provides strategic and operational leadership for care coordination and case management activities within the Population Health Program, advancing CFHC, Inc.’s patient-centered team-based care model.
The Director ensures that high- and moderate-risk patients are systematically identified, stratified, and engaged in services designed to improve health outcomes, enhance quality of care, and reduce overall health care costs. This role is accountable for establishing measurable program goals, monitoring performance metrics, and driving continuous improvement while maintaining alignment with CFHC, Inc.’s strategic priorities.
The Director fosters collaboration among population-specific program leads and cross-functional departments to ensure coordinated, organization-wide population health initiatives.
THIS POSITION OFFERS A 40-HOUR WORKWEEK, NO WEEKENDS, FULL BENEFITS PACKAGE, 12 PAID HOLIDAYS, BI-WEEKLY PAY.
Essential Tasks Provides leadership and oversight to the Population Health Programs and teams.
Ensures development of Population Health initiatives to improve patient satisfaction and family centered care.
Provides direct supervision to the Population Health Managers, Pediatric Program Manager, and Care Coordinators. This includes but is not limited to interviewing, hiring, providing training and orientation, distribution and determination of responsibilities, and the on-going evaluation of the staffs’ performance.
Reports operational monitors and productivity for staff and perform audits to monitor adherence with CFHC, Inc., Medicare/Medicaid, and other regulatory agencies rules and regulations for providing and documenting case management services and address deficiencies in a timely manner.
Oversees care coordination and case management activities including workflow development, use of the electronic health record, and case management systems for documentation and reporting.
Provides case management/chronic care management and transition of care services to patients on an as needed bases to cover staff shortages or call outs. Conducts annual comprehensive risk assessments and develop patient-centered care plan for patients. Reassess the care plan based on corporate policy. Effectively communicate care plans and other client-related activities both verbally and in writing to the patient and care team.
Experience and Education
Bachelor of Science in Nursing (BSN) or an Associate Degree in Nursing (ADN) combined with a bachelor’s degree in a health-related field.
Minimum of three years of experience as a Registered Nurse or other licensed healthcare professional, including at least one year of experience in population health, case management, and at least one year supervisory or management role.
North Carolina Licensed Registered Nurse. BLS Certification. Valid NC Driver License.
Schedule 8-hour shift

Physical Requirements
Individuals may need to sit or stand for long periods of time, occasional bending, stooping, lifting including of patients, reaching forwards and overhead.
Possibly requires walking primarily on a level service for periods throughout the day.
Both proper lifting techniques and frequent computer work are required.
Must be able to travel freely in the local area; between clinics, hospitals, health departments, etc.
