Social Services Director , you’ll play a vital role in supporting residents’ emotional well‑being, care planning, and successful transitions while advocating for dignity, respect, and quality of life.

What You’ll Do
Develop, plan, implement, and evaluate the Social Services program in accordance with federal, state, and center guidelines
Identify and address residents’ social, emotional, and psychological needs
Coordinate services to support resident adjustment, care planning, and discharge needs
Collaborate with interdisciplinary teams to ensure holistic, person‑centered care
Assist residents and families with resources, education, and care transitions
Maintain accurate documentation in compliance with regulatory standards, including MDS‑related processes
Advocate for Residents’ Rights and ensure concerns are addressed promptly and compassionately
Support compliance with Medicare, Medicaid, and other reimbursement and eligibility guidelines
What You Bring
Licensed Social Worker (required)
At least one year of supervised social work experience in a healthcare setting, preferably working with geriatric populations
Working knowledge of nursing home regulations, MDS procedures, and reimbursement guidelines
Strong written and verbal communication skills
Compassionate, service‑oriented mindset with a desire to support residents and families
Positive, dependable, and team‑focused attitude
Excellent attendance and reliability
