Pay or shift range: $75,000 USD to $80,000 USD The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and other job-related reasons.

Description The

Assistant Director of Wellness , in coordination with the Director of Wellness, is responsible for the day-to-day management and success of The Bristal at Lake Success’ Wellness Department and Reflections Program. These responsibilities include ensuring excellence in Resident care and programming, providing outreach and support to families and acting as liaison between Residents, their families, administration staff and community-based service providers.

Schedule: Tuesday - Saturday 9am to 5:30pm

DUTIES AND KEY RESPONSIBILITIES:

At the beginning of every shift, walk the entire unit to ensure that there are no problems, incidents nor issues that need immediate attention.

Help maintain personnel and Resident files.

Oversee Medication assistance by trained medication aides. Monitor closely procedures of medication supervision, medication records, medication re-ordering and ensure medication accountability.

Direct Resident Service Aides in all aspects of care.

Assess Residents regularly to establish their needs and to establish and update personal care plans. Coordinate care with the Executive Director, Director of Case Management and Director of Wellness.

Schedule and conduct regular in-services to improve the quality of care by Reflections personnel.

Maintain in-service schedule to meet Department of Health requirements.

Ensure that monthly weights are taken and recorded.

Supervise scheduled checks of Resident’s rooms.

Meet regularly with the Director of Wellness to exchange information about the Residents and personal care services.

Assist in ensuring that monthly calendar of activities is being adhered to and ensure RSA presence.

Relate to and give support to family members on an ongoing basis.

Enter resident case notes in the event of an incident.

Act as Resident advocate and assists administrative staff in their continued service to the Resident.

Maintain active community and professional relationships to support positive promotion of company.

Assertively greet family members when they enter the unit.

Communicate with sponsors and families regarding health needs and status of Residents and provide documentation in case notes.

Provide input regarding admission, retention and discharge of Residents.

Assist in conducting visits with residents during the hospitalization/rehab at the outset of the stay and throughout as warranted.

Screens, interviews, hires, trains, evaluates, counsels, disciplines and terminates departmental personnel.

Follow all workplace injury procedures, including completion of reports for timely reporting and issuing corrective action when necessary.

Schedule staff assuring that one RSA on each shift has basic First Aid Certification.

Implement and supervise the systems for maintaining accurate records and documentation that meets all requirements of the facility and DOH regulations.

Establish and maintain a medication system that adheres to Federal, State, local Department of Health laws and regulations and company policies.

Communicate with attending and specialized physicians regarding Resident health needs, communicate outcome of physician visits to families and document the same in case notes.

Ensure staff accurately complete incident report records in compliance with Department of Health Regulations and company policy.

Adhere to and uphold company policies and procedures.

Performs other duties as assigned by the Executive Director.

QUALIFICATIONS:

Graduate from an accredited school of nursing or 2 years’ experience in an Alzheimer environment.

Minimum 2 years prior experience in a geriatric setting, preferable residential.

Minimum of 2-3 years supervisory experience.

Knowledge of geriatric care needs, supervision and administration.

Knowledge and performance on in-service procedures.

Use of compassion, tact and courtesy relating with Residents, employees and families.

Excellent eyesight with or without correction.

Acute sense of smell.

Knowledge of Microsoft Office and email.

Must read and speak English.

Must be able to tolerate frequent work interruptions, organize work and reset priorities in order to complete work responsibilities in a timely manner.

Must be able to cope with the mental and emotional stress related to working with persons who elderly and/or emotionally upset.

Must be able to listen attentively to residents', requests and preferences, ensuring exceptional customer service.

Must be able to speak, read, and write in English, which is the predominant language of a majority of our residents.

Must be emotionally, mentally and physically able, with or without reasonable accommodations, to provide required services to residents, including verbally communicating with residents.
