About At Home Care Partners At Home Care Partners is a well‑established Licensed Home Care Services Agency (LHCSA) in New York. We are seeking an experienced Registered Nurse to serve as Director of Patient Services (DPS) and lead our clinical operations.
Position Overview The Director of Patient Services (RN) is an executive‑level leadership role responsible for overseeing clinical operations and supporting general agency operations to ensure high‑quality patient care. The DPS provides clinical oversight, ensures regulatory compliance, and leads caregiving, nursing, and patient services teams, including oversight of Personal Care Aide (PCA) training and certification programs. The role also supports organizational growth through strong relationships with healthcare partners and referral sources.
The ideal candidate is an experienced Registered Nurse with a strong background in home care and solid knowledge of New York State Department of Health (NYSDOH) regulations.
Key Responsibilities
Provide overall leadership for patient care services and day‑to‑day clinical and operational performance across caregivers, nurses, and patient services teams.
Ensure compliance with NYSDOH/CMS requirements, HIPAA, and agency policies; maintain survey readiness and lead audits/inspections, including corrective action plans as needed.
Lead, mentor, and evaluate clinical and support staff; maintain appropriate staffing levels and oversee scheduling, field supervision, and performance monitoring.
Develop and implement clinical policies, procedures, and process improvements to support quality, consistency, and efficient workflows across departments.
Oversee patient intake, RN assessments, and interdisciplinary care planning, coordinating closely with physicians, caregivers, and internal teams.
Ensure timely and accurate clinical documentation, reporting, and regulatory submissions in accordance with DOH standards.
Oversee PCA training and certification programs, including curriculum oversight, instructor supervision, documentation, and compliance with NYSDOH requirements.
Support organizational growth by building referral relationships and partnerships and contributing to marketing/outreach efforts.
Qualifications
Current New York State Registered Nurse (RN) license (required).
Minimum of 2 years of clinical nursing experience, including at least 1 year in a home care related role (required).
Prior experience as a Director of Patient Services or clinical manager in a CHHA or LHCSA setting strongly preferred.
Strong knowledge of NYSDOH and CMS home care regulations (required).
Bachelor’s degree in Nursing (BSN) required.
Excellent leadership, communication, and organizational skills with a strong focus on quality and compliance.
Proven ability to develop and implement clinical policies and to manage and mentor a multidisciplinary care team.
Proficiency with electronic medical records and home care documentation systems.
