We are seeking a Social Services Specialist Rover to join our regional team. You will be the "clinical hero" who steps in to provide essential support across multiple nursing centers in our region. Whether you are covering a leave of absence, supporting a high‑census period, or ensuring a nursing center meets crucial regulatory standards, your expertise will ensure our residents receive the highest level of psychosocial care.

Position Highlights
As a Social Services Specialist Rover, you serve as a champion for resident rights and mental well‑being. Your core mission is to provide a seamless psychosocial experience through the following:
Clinical Advocacy: Conducting comprehensive assessments and developing interdisciplinary care plans that honor the strengths and needs of every resident.
Navigational Support: Acting as the lead architect for discharge planning by connecting residents with community resources and ensuring a safe, confident transition home.
Crisis & Transition Management: Providing therapeutic interventions to help residents and families navigate the emotional journey of long‑term care.
Regulatory Excellence: Ensuring all documentation (MDS, Social Histories, and Care Plans) is precise, timely, and compliant with NASW and government standards.
Collaboration: Working in collaboration with nursing, therapy, and administration to provide a holistic approach to health.
Qualifications
Bachelor's degree from an accredited school of Social Work.
Must possess any certifications/licensures as required by the State of employment to practice in long‑term care.
One to three years of supervised social work experience in a health care setting working directly with individuals preferred.
Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse preferred.
