Job Summary

Position title: Associate Director of Enrollment, Care Management Division

The Associate Director of Enrollment, Care Management will lead and oversee all enrollment operations for Essen Health Care’s Care Management and Health Home programs. This role is responsible for managing enrollment strategy, ensuring regulatory compliance, optimizing workflows, and supporting growth initiatives across partner organizations.

The ideal candidate is a results-driven operational leader with deep knowledge of NYS Health Home enrollment requirements and the ability to scale enrollment processes while maintaining accuracy, compliance, and high service standards.

Responsibilities

  • Growth & Strategy
    • Develop and execute a comprehensive growth strategy for Essen’s Care Management services across New York
    • Identify new market opportunities, partnership models, and service lines to expand the care management footprint
    • Build and manage a robust pipeline of partner practices and healthcare organizations
    • Analyze market trends, payer dynamics, and regulatory changes to inform strategic planning
    • Drive innovation in care management delivery models aligned with population health and value-based care initiatives
  • Partnership Development
    • Identify, establish, and maintain strategic partnerships with primary care practices and healthcare organizations
    • Negotiate and execute partnership agreements that align with organizational goals and ensure mutual benefit
    • Serve as the senior relationship owner for partner organizations, ensuring strong engagement and performance
    • Develop scalable partner recruitment and onboarding strategies to support growth targets
  • Operational Excellence
    • Oversee care management operations across the partner network, ensuring consistent service delivery and quality standards
    • Ensure regulatory compliance with NYS Medicaid managed care and care management requirements
    • Develop and implement scalable processes for partner onboarding, training, and ongoing support
    • Monitor performance metrics and implement continuous improvement initiatives
  • Financial Performance
    • Own revenue growth and profitability targets for the Care Management business line
    • Monitor financial performance across partnerships and identify opportunities for optimization
    • Develop pricing strategies, budgeting processes, and cost management initiatives
    • Provide regular business performance reporting to executive leadership
  • Leadership & Collaboration
    • Build, develop, and lead a high-performing, cross-functional team
    • Foster a culture of accountability, innovation, and continuous improvement
    • Collaborate with clinical, finance, compliance, legal, and operational leadership across the organization
    • Serve as a key strategic advisor to executive leadership on care management growth initiatives

Qualifications

Required Qualifications

  • 7+ years of progressive healthcare leadership experience in care management, population health, or value-based care
  • Proven track record of driving business growth and strategic partnerships in healthcare
  • Strong understanding of NYS Medicaid managed care and care management regulations
  • Demonstrated P&L ownership and financial accountability
  • Exceptional negotiation, communication, and relationship management skills
  • Master’s degree in Business Administration (MBA) or related field strongly preferred

Preferred Qualifications

  • Deep knowledge of the NYC/Bronx healthcare market and provider landscape
  • Experience structuring and managing healthcare services agreements
  • Background in scaling multi-site healthcare or care management operations
  • Experience leading growth initiatives in complex healthcare environments
