Job Summary\nThe Medical Billing & Coding Specialist assures accurate and complete coding information is collected and reported to private insurance and Medicare to help complete the revenue cycle. The specialist will scrub encounters for accurate coding prior to claim creation, assure correct modifiers and ICD10 diagnosis codes are allocated to each CPT code, ensure timely claim submissions and follow-up on claim denials. The candidate should have knowledge of insurance regulations and medical coding with the goal of maximizing accurate third-party billing and minimizing denials. Duties and Responsibilities

Accurate and timely submission of medical claims to insurance companies and other payers

Review and analyze medical records to ensure appropriate coding of diagnoses and procedures

Document for providers and management any insufficient or unclear information on claims

Assign or reassign CPT, HCPCS, and ICD-10-CM codes as needed

Follow up on unpaid claims and initiate appeals for denied claims within 30 days of submission.

Track the progress of claims through the clearinghouse and promptly address any issues

Resolve patient billing issues and questions via phone and email in a timely fashion

Stay updated on healthcare regulations, medical terminology, and coding practices

Follow HIPAA guidelines when accessing and sharing patient information

Additional job related duties or projects as needed

Qualifications and Skills

Minimum of 3 years’ experience with medical billing or revenue cycle in a medical setting

Certified Professional Coder thru AAPC or a Certified Coding Specialist thru AHIMA - Required

Knowledge of insurance guidelines including HMO/PPO, Medicare and other payers’ requirements and systems

Knowledge of CPT, ICD-10, HCPCS Coding and utilization of modifiers

Knowledge of medical billing rules, modifiers, and strong understanding of EOBs and ERAs

Competent in computer skills, Microsoft Office or similar software

Experience with

AthenaHealth

EHR is preferred or other similarEHR systems such as

Experience with Family Practice and Primary Care outpatient billing (Preferred)

Exceptional Customer Service skills for interacting with patients regarding medical claims and payments

POSITION SUMMARY\nUnder general supervision the position is responsible for billing, coding, posting, assigning correct payer sources, submitting clean claims to insurances and follow up for on denial claims in order to maximize the revenue cycle through billing and coding expertise. Reviews, analyzes and assures the final diagnoses and procedures as stated by the practicing providers are valid and complete. Train and educate staff regarding proper billing and coding practices.

MINIMAL QUALIFICATIONS

High School Diploma

Medical Coding Certificate – CPC or CCS certification required

Excellent interpersonal skills

Two years’ experience using ICD coding, CPT, HCPS or equivalency

RESPONSIBILITIES AND PERFORMANCE EXPECTATIONS

Assesses adequacy of health record documentation in order to support accurate, complete and specific assignment of modifiers, International Classification of Diseases (ICD), Current Procedural Terminology (CPT) and Health Common Procedural Coding (HCPC) codes to maximize reimbursement.

Reviews record to ensure that the appropriate billing provider, required attachments, signatures and other relevant documentation to reflect treatment and services rendered are present as required.

Properly assigns and corrects procedure, modifier and diagnosis codes supported by provider documentation. Appropriately queries provider for clarification or additional documentation needed for processing a clean claim.

Generates and processes claims from provider documentation to submit claims to appropriate payer in accordance with Medicare, Medicaid and Managed Care policies for proper reimbursement.

Researches and ensures corrections are made on denied claims due to missing or incorrect information. Follows up with appropriate party as warranted regarding denials and payments.

Reviews claims in holding status to process claims in a timely manner.

Provides recommendations to Director of Revenue Cycle Management regarding accounts receivables, coding and billing practices.

Attends seminars and in-services as required to remain current on coding issues.

Maintains all mandatory in-services.

Maintains compliance standards in accordance with the Compliance policies and the Code of Conduct. Reports compliance problems appropriately.

Determines the final diagnoses and procedures stated by the physician or other health care providers are valid and complete.

Keeps up with federal laws and regulations affecting coding requirements.

Demonstrates principles, practices and methods of current coding certification required.

Follows modern/best office practices.

Possesses knowledge of billing practices required, FQHC preferred.

Has knowledge of medical records, EHR required.

Has extensive knowledge of official coding conventions and rules established by the American Medical Association (AMA), and the Center for Medicare and Medicaid Services (CMS) for assignment of diagnostic and procedural codes.

Makes up good math skills and effective communication skills.

Performs coding work requiring independent judgement with speed and accuracy.

Examines and verifies coding errors through audits.

Completes required in-services.

Communicates clearly and concisely, orally and in writing.

Maintains confidentiality.

Can use the computer effectively.

Works independently to accomplish assigned work in a timely manner.

Communicates with staff and the public, both in person and over the phone, in a tactful manner and under difficult situations.

Understands and carries out verbal and written directions.

Follows CFHC policies and procedures.
