This role requires Florida AHCA Clearinghouse background screening. https://info.flclearinghouse.com Job Duties as a Transitional Care Coordinator:
Develop relationships specific to area to assist in maximizing census
Increase referral rate from area hospitals
Coordinate post-discharge care services and resources, including home health services, durable medical equipment and follow up appointments
Provide education to patients and families on post discharge instructions
Serve as a liaison between patients, families, and healthcare providers to resolve issues and overcome barriers to care
Maintain accurate and up-to-date documentation of transitional care plans, patient education activities, coordination efforts, and outcomes
Monitor and evaluate the effectiveness of transitional care interventions, making adjustments as needed to optimize patient outcomes
Maintains knowledge of current regulations governing Medicare, Medicaid Insurance
Embrace company's core values and incorporate them into daily job
Demonstrate respect and compassion in every interaction
Perform other Duties as assigned
Conduct comprehensive assessments of patients medical, social and support needs to develop individualized transitional care plan
Qualifications for Transitional Care Coordinator:
2 years of relevant experience in Health Care Marketing or Business Development
Bachelors Degree
Must be 18 years or older
Ability to deal tactfully with personnel, residents, family members and the public
Exceptional people skills
Excellent verbal, written and interpersonal communication skills
Ability to work independently
Must have good time management
Ability to prepare accurate reports
Must be proficient in the use of a computer
Have knowledge of healthcare systems, community resources and discharge planning processes
Must have empathy, compassion and patient centered approach to support patients in transitions of care
