Restore Independence: Conduct initial evaluations and developfunctional, goal-oriented care plans.
Clinical Excellence: Provide one-on-one therapeutic interventions,including gait training, strengthening, and manual therapy.
Safety First: Perform home safety assessments and recommendmodifications to prevent falls.
Documentation:
Complete timely OASIS assessments and progressnotes via our intuitive EMR system.
Collaborate:
Work closely with physicians and theStellarCare clinical team to ensure seamless patient transitions.

A degree in Physical Therapy from anaccredited program and a valid state license.
Experience:
