The Opportunity Leaves Personal Care is seeking a

Marketing & Community Outreach Coordinator

to lead our visibility, relationships, and growth throughout West Michigan.

This role exists to connect families in need with exceptional private-duty home care, and to do so in a way that reflects our faith-based foundation and deep commitment to dignity, advocacy, and relationship-centered care.

The Core Purpose of This Role To generate qualified inquiries and referrals for private-duty home care, and to guide those relationships from first contact to ongoing service, while representing Leaves’ unique values and commitment to dignified, personalized care at home.

What You’ll Be Responsible For: Community & Referral Outreach Plan and execute consistent outreach to hospitals, rehab facilities, assisted and independent living communities, senior centers, churches, case managers, financial planners, and other community partners Build and deepen referral relationships through drop-ins, value-added materials, group presentations, and follow-up meetings Maintain warm, ongoing contact so key relationships never go “cold”

Sales & Client Engagement Conduct in-person intakes with prospective clients and families Assess care needs, explain services and pricing clearly, and guide families through the decision and onboarding process Maintain relationships with existing clients and families to support satisfaction, trust, and continued referrals

Digital Marketing & Brand Presence Manage and maintain a social media calendar and posting cadence Support website content updates, local SEO efforts, and basic analytics review Coordinate with leadership or vendors on any paid advertising efforts Ensure all digital and written communication reflects Leaves’

and is accessible to seniors and family decision-makers

Events & Community Representation Represent Leaves at community events, health fairs, church programs, senior networking events, and local fundraisers Participate in occasional evening or weekend events to strengthen brand visibility and community trust

Tracking & Accountability Track outreach, leads, referrals, conversions, and activity in a CRM or tracking system Provide weekly reporting on activity, pipeline, and results Report to the Operations Director to align marketing efforts with growth goals and budget considerations

How Success Is Measured This role has clear, achievable performance markers, including:

Lead Generation

Consistent flow of qualified inquiries from phone, web, referrals, and community outreach New referral partners opened each quarter who begin sending active referrals

Conversion & Growth

Strong conversion rate from inquiry to signed service agreement Contribution to increased weekly billable hours and overall revenue growth

Relationship Management

Steady cadence of meaningful outreach and visits with key referral partners Up-to-date contact records and documented touchpoints for top relationships

Brand Integrity

All messaging and interactions reflect Leaves’ values of faith, dignity, advocacy, and excellence Professional, warm, and trustworthy presence in all settings

Role Requirements Prior experience in healthcare or relationship-based sales/outreach strongly preferred Strong interpersonal communication, listening skills, and empathy Comfort managing social media, basic SEO concepts, and simple digital tools Ability to create clear, professional marketing materials (flyers, presentations, basic Canva designs) Reliable transportation, valid driver’s license, and willingness to travel locally
