Job Description Summary An Enrollment Specialist is responsible for guiding individuals through the enrollment process in various sectors, ensuring a smooth experience while managing applications and maintaining accurate records. The Enrollment Specialist is Eldercare's prospect liaison. This person generates and works referral lead from the Outreach Liaison. He/She will develop a relationship-based approach to cultivating interest in the program through initial contact and routine, purposeful follow up. Communicating Eldercare PACE’s value proposition with urgency to each prospective member. The Enrollment Specialist assists with the enrollment process, collaborates with referral sources, assists prospective participants to obtain Medicaid eligibility, an approved Level of Care, and collaborates with staff internally.
Position Responsibilities
Responsible for obtaining all required documentation to begin enrollment process, guiding individuals through the enrollment process, ensuring a smooth experience while managing applications and maintaining accurate records.
Responsible for timely follow up on incoming referrals from Outreach Liaison team members via their marketing efforts.
Responsible for meeting patients, families and others to provide education on the PACE model of care and Eldercare program services and invite those that qualify to enroll in Mount Sinai Eldercare's health plan.
Expected to meet or exceed monthly targets and annual goals for call-ins/outs, conversions to participants.
In conjunction with the Dir. Of Business Development and Outreach liaison team develop annual, quarterly and weekly plans that reflect appropriate daily sales calls/staff/patient interactions to educate and promote the Eldercare program to meet enrollment goals.
Maintains accurate account list for all prospective referral sources and tracks all participants through the enrollment process in the CRM.
Working closely with Enrollment Coordinator to ensure enrollment completion of prospective members.
Attends and participates in Eldercare enrollment meetings and other meetings as directed.
Escalate problems or concerns to the Director of Business Development, and check in with prospects to ensure their questions have been fully answered.
Maintains a working knowledge of applicable Federal, State, and local laws and regulations, including Mount Sinai and Eldercare policies on compliance, Fraud Waste and Abuse, Code of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
Manages documentation, processing applications, verifying documents are completed and submitted correctly, inputting data accurately into the CRM and EHR timely.
Provides information timely on all new enrollees as needed to ensure the enrollment process runs smoothly and participant and referral source are satisfied with the process.
Communicates timelines for prospective participant enrollment status within the enrollment department and Business Office.
Assists with the financial eligibility process for potential PACE participants by reviewing Medicaid eligibility and assisting prospects with filing the Medicaid documentation including all of the necessary supporting documentation for approval.
Ensures necessary paperwork related to participant enrollment is received by the Business Office on or before the appointed cut-off date.
Participates in the quality improvement program, as needed, and any special projects assigned by the Director of Business Development.
Provide feedback to the Director of Business Development of any process that can be modified or improved in the daily process.
Attend to all trainings per PACE regulations related to functions of enrollment and customer service.
Participate in outreach events and information sessions to promote Eldercare PACE and its services to potential participants.
Qualifications
License/Registration/Certification
BLS or CPR Certified.
Education
Graduated from an accredited school of nursing (LPN), social work (BSW) or Bachelors degree in health care or marketing related field is preferred. High School or graduation equivalent is required. Education in geriatrics is also preferred.
Experience
Two (2) years of sales or marketing experience is preferred in a health care setting. A combination of education and experience may be acceptable. At least one year working with the frail or elderly.
