Role Description The Director of Value Based Care is responsible for overseeing the implementation and management of value-based care programs to improve patient outcomes, enhance care coordination, and optimize cost efficiency. Key duties include developing and evaluating care models, managing interdisciplinary teams including clinicians and case managers, and ensuring compliance with regulatory requirements. This is a full-time, on-site role based in Florence, SC.
Qualifications
Skilled at big-picture strategy development and practical implementation
Strong background in analytics and data management
Thorough knowledge of clinical quality metrics
Experience in general clinical practice
Strong initiative, highly self motivated and outcomes driven
Proven ability in Training and leadership development
Demonstrated experience in implementing and managing value-based care initiatives
Excellent analytical, decision-making, and organizational skills
A master’s degree in Healthcare Administration, Nursing, or a related field is preferred
Familiarity with Federally Qualified Health Centers and population health management is a plus
