is seeking a compassionate and experienced
Social Services Director
to lead psychosocial services and support resident-centered care in a long-term care setting. This leadership role is ideal for a professional who is passionate about resident advocacy, care coordination, and supporting both residents and families throughout their stay. Key Responsibilities Oversee and direct all social services operations within the facility Conduct psychosocial assessments, care planning, and required documentation, including MDS input Advocate for residents emotional, social, and mental well-being Collaborate with the interdisciplinary team to support care conferences and care transitions Assist residents and families with discharge planning, transfers, and community resources Maintain compliance with state and federal regulations related to social services Build strong relationships with residents, families, and community partners Support resident engagement initiatives and quality-of-life programming
Qualifications
Bachelors degree in Social Work or related field preferred Current or eligible licensure in the state of Colorado (LSW/LCSW preferred) Prior experience in skilled nursing or long-term care strongly preferred Working knowledge of MDS, care planning, and regulatory requirements Strong communication, organizational, and leadership skills
