Doctor of Optometry (OD) from an accredited optometry school
Valid state optometry license
MD or DO from an accredited ophthalmology school
Valid state ophthalmology license
General liability certificate / professional liability insurance certificate
ME/VA license agreements are with Gumberg Asset Management Company.
