This position functions as a “Floater” and will travel or use telehealth to provide services between health centers as needed.

Plans and manages comprehensive nutritional care component of a large, designated caseload of clients in the County Health Department primary care services including maternity, family planning clinics and HIV/AIDS. Conducts nutrition assessments and individual counseling, Medical Nutrition Therapy on normal and therapeutic diets. Provides nutrition consultation for contracted agencies. Provides group nutrition in specialty clinics. Educates clients on glucose self‑monitoring. Coordinates any assigned food assistance programs. Documents client contacts and services in the Electronic Health Record. Participates in client case‑management conferences with other health center staff.

Coordinates and evaluates the diabetes care program for the Comprehensive Nutrition Program.

Researches current American Diabetes Association guidelines for diabetes, including gestational diabetes and assists the supervisor in designing protocols and procedures. Coordinates nutrition education programs for community agencies at worksite locations through presentations, health fairs and community nutrition classes. Identifies and develops nutrition resources for community projects. Interprets current scientific information of food, nutrition, diet and health to professionals, agencies and the public.

Assists with coordinating the Therapeutic Nutrition Treatment (TNT) food assistance program for HIV/AIDS clients. Manages program design, food procurement, inventory control and EHR documentation. Coordinates TNT program with Medical Nutrition Therapy services and follows all critical control procedures and guidelines established by HRSA and the Ryan White Program. Monitors and evaluates TNT Program with nutrition staff.

Performs blood glucose testing according to Palm Beach CHD Laboratory Procedure Manual and DHM 150‑24 as required. Performs finger sticks to obtain blood sample using universal precautions. Cleans and sanitizes blood collection area and equipment.

Evaluates effectiveness of nutritional care. Participates in quality improvement activities. Summarizes and analyzes nutrition activities and services at regular intervals.

Serves as consultant and resource person on nutrition for health department staff. Assists in planning and supervising field experience for nutrition students, interns and medical residents. Participates in staff meetings, in‑service training and other staff development activities. Maintains records and prepares reports.

Plans and/or participates in outcome studies and surveys on the nutritional aspect of health and disease. Collects and maintains data relating to studies and surveys. Assists in data analysis and prepares summarizing data.

Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty includes working in special needs shelters or performing other emergency duties related to disaster response.

Performs other duties as assigned.

Qualifications Licensure/registration/certification requirements: Must meet the requirements outlined in Chapter 468, Part X, Florida Statutes, or be eligible to practice dietetics in accordance with Chapter 64B8‑40 to 45, Florida Administrative Code and/or Registration as a dietitian as defined by the American Dietetic Association.
