POSITION SUMMARY The Outreach and Enrollment Specialist’s role is to primarily raise awareness about TCA Health Center and the services we provide. The Outreach and Enrollment Specialist will provide eligibility and enrollment assistance to uninsured patients of TCA Health Center and residents in their approved service areas. They will also work with the Outreach team to carry out the strategic outreach plan with a focus on engaging/attracting new patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES (include but not limited to the following);

Work with the Patient Service Coordinator to identify existing patients who are in need of access affordability options. Also to provide any updates on the status of recruitment and enrollment.

Provide a weekly report on the status, issues and successes with recruitment and enrollment.

Perform their responsibilities and duties assigned by management, along with the listed duties above.

Perform as an advocate for patients, monitor the quality of services and service delivery.

Perform outreach activities with community-based organizations and schools.

Ensure patient access to services as prescribed by their health care providers.

Distribute health information and health education to patients.

Assess needs of clients in assigned community, refer to case manager and follow through on client needs.

Conduct questionnaires for door-to-door survey throughout the assigned community.

Provide input into health education workshops.

Develop strong working relationships with the appropriate organizations in the service areas.

Schedule and link participants to TCA Health, Inc. for primary care services.

Follow up on case finding attempts via phone, mail and/or home visits and document necessary information in EMR as needed.

Make appropriate referrals to community agencies.

Prepare daily reports of outreach assignment results.

Engaged in outreach strategies under the direction of the Program Coordinator.

Attend program team and staff meetings.

Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards and participate in all Patient Centered Medical Home efforts.

Conduct monthly public education activities to raise awareness about coverage options available under Medicaid, CHIP and the Marketplace

Provide information and assistance in a fair, accurate, and impartial manner to all interested residents and in reach patients.

Provide information and assistance in a manner that is culturally and linguistically appropriate to diverse communities and accessible to individuals with disabilities.

QUALIFICATIONS

Able to initiate and maintain working relationships.

Have understanding and knowledge of population and community of service area.

Able to manage time and set priorities.

Good community skills; able to listen well, use language appropriately, and good writing skills.

Exhibits excellent guest relations with patients, customers, visitors and staff effectively.

EDUCATION AND EXPERIENCE

High School diploma or GED equivalent and a minimum of two years community experience working with uninsured and underinsured clients.

Must be knowledgeable about the community and have a good relationship with the community leaders and residents.
