Quality of Care and Nursing Process The nurse provides evidence-based care to prevent Healthcare Acquired Conditions and hospital readmissions. The nurse practices standard precautions during patient contact and while handling contaminated materials. Assessment Nursing assessments are completed according to hospital and unit specific policies, procedures, and standards of nursing care. Collects pertinent data from the patient, significant others, and health care providers. A comprehensive patient history and physical is completed in a timely manner and includes biophysical, psychosocial, environmental, self-care abilities, educational, age-specific, cultural, and discharge planning needs. Diagnosis / Care Needs Nursing diagnoses / care needs are derived from the assessment data. Involving the patient and significant others as appropriate, the nurse formulates a plan to provide appropriate care based on identified patient needs. Implementation Nursing care is based on identified nursing diagnoses / care needs and patient care standards, and is consistent with therapies of other disciplines. Nursing care is based on identified nursing diagnoses / care needs and standards of patient care. Evaluation Evaluation of the patient’s progress towards attainment of desired outcomes is evident in ongoing assessment data. Nursing diagnoses / care needs are revised based on reassessment data. Education:
Associate Degree in Nursing or graduate of a Nursing Diploma program. Certification/Licensure: Current, valid Arizona RN license or compact state RN license Current BLS certification from American Heart Association or American Red Cross Unit Specific Requirements
Medical Surgical Units: Basic EKG competency within 3 months of hire Pediatrics: PALS certification from American Heart Association or American Red Cross required within 6 months of hire Intensive Care Unit / Progressive Care Unit: ACLS certification from American Heart Association or American Red Cross required upon hire Basic EKG competency within 30 days of hire Women’s Services: NRP certification required within 6 months of hire/transfer ACLS within 3 months of hire/transfer S.T.A.B.L.E. required within 6 months of hire/transfer AWHONN Fetal Monitor Course: Basic course within 6 months; intermediate course within 12 months Emergency Department: ACLS and PALS within 3 months of hire Emergency Severity Index (ESI) training within 6 months of hire TNCC within 24 months of hire Hemodialysis: ACLS Basic ECG within 3 months of hire Clinical Decision Unit: Basic EKG competency within 3 months of hire ECHO: ACLS Basic ECG competency within 30 days of hire Surgical Services / Circulator: ACLS and PALS within 90 days of hire Basic EKG competency within 30 days Minimum five years of experience for full-time shift with weekend/evening/night; minimum one year in specified areas for seasonal hires; no call for full-time weekend/evening hires Surgical Services / PACU / Outpatient: ACLS and PALS within 90 days; 1+ year acute care or med-surg; 30 days EKG; 5 years for full-time weekend/evening shifts; no call for full-time weekend/evening shifts Cath Lab: ACLS; ECG competency; 2+ years RN experience Cardiology: Basic EKG competency within 30 days Clinical Lead: Maintains BLS/ACLS/PALS, TNCC, de-escalation, NRP, Basic EKG, and ESI as required per RN position description for assigned area(s).
