This role is based in an Assisted Living, Independent Living, and Memory Care setting. This position may be offered as a Full-Time or PRN opening, depending on several factors. Please apply, and a member of our team will reach out to discuss available options.

Summary:

Provide Occupational Therapy and related services for patients under the direction of a licensed Occupational Therapist (OT) while maintaining positive levels of interaction with communities and clients in accordance with the principles and practices of occupational therapy and within Reliant Rehabilitation guidelines. Provide quality patient care including ongoing treatment per a plan of care, supervision of Rehab Techs (where required), and case management responsibilities. Maintain professional conduct that represents Reliant Rehabilitation’s commitment to quality and service in all interactions with clients, including positive levels of interaction and rapport with communities and clients. Adhere to all state and federal regulations regarding occupational therapy.

Responsibilities

Under the direction of the licensed occupational therapist, optimize the functional abilities and skills of patients in communities.

Assist in screening and provide input to the initial evaluation of patients as appropriate.

Implement treatment for all assigned patients.

Comply with state and federal guidelines regarding accurate and timely billing and documentation.

Establish and maintain effective interactions with patients, interdisciplinary team, and other clientele reflecting patient-centered care.

Provide direct supervision to rehab techs during group and individual treatment to promote appropriate resource utilization and enhance patient quality of care, following state practice acts, federal regulations, and Reliant Rehabilitation guidelines.

Build a strong occupational therapy assistant program by participating in activities such as integrating OT’s role into community and interdisciplinary treatment plans, contributing to community patient care, utilization review, case management, administrative staff/department head, and family conference activities and communications, as directed by the OT.

Actively integrate occupational therapy’s role into community and interdisciplinary treatment plans.

Contribute to community patient care, utilization review, case management, administrative staff/department head, and family conference activities and communications.

Establish and maintain strong working relationships with other health care professionals and colleagues in the community.

Make appropriate referrals to other rehabilitation team members.

Maintain awareness of changes in patient population, conditions, and programs, and keep the licensed or registered OT informed of patient progress and community issues in a timely manner.

Adhere to community policies and procedures related to environmental safety, orderliness, risk management, infection control, and emergency preparedness.

Adhere to patient rights and privacy requirements.

Enhance clinical expertise and professional and management skills through interaction with managers, therapists, other staff, self-study, and continuing education.

Maintain a valid state license (where appropriate) and stay informed on issues related to the OT profession and the healthcare environment.

Understand relevant payment models related to billing and treatment guidelines, and provide clinically appropriate care with reasonable goals and outcomes within the guidelines.

Maintain compliance with infection control procedures and environmental safety protocols.

Perform patient lifting using biomechanical techniques throughout the shift; able to lift fifty (50) pounds of dead weight alone.

Maintain confidentiality of Reliant Rehabilitation, community, and patient information.

Act ethically, legally, and responsibly at all times.

Adhere to policies, principles, and guidance in the Team Member Handbook and Code of Business Conduct.

Travel between communities as scheduled.

Work varying hours and shifts, including weekends and holidays.

Attend all mandatory meetings, trainings, and assignments as delegated.

Perform other duties and responsibilities as assigned.

Qualifications Competency:

Prior to beginning patient care for Reliant Rehabilitation, a peer OT may complete a Competency Checklist.

Education/Experience:

Graduate of an ACOTE accredited school of occupational therapy.
