Job Summary The Central Intake Admissions Specialist works with a sense of urgency as part of a self-empowered team to drive census through developing relationships with referral sources and location leadership. Assesses potential referrals for financial viability and clinical needs alignment with location capabilities. Strives to meet or exceed Speed to lead metrics.
Primary Duties and Responsibilities
Timely retrieval/response to information from referral platforms and sources
Enter all referrals into CRM in PCC: demographic and insurance information; attach referral packet from hospital
Evaluate each referral for Clinical and Financial Viability for admission based on Red/Green/Yellow light process and take appropriate action based on process parameters
Develop and maintain strong relationships with all internal and external customers to yield an increased share of satisfied patient referrals from targeted referral partners
Effectively communicates each referral with the Heath Care Navigator.
Complete or ensure completion of the Quick Patient Assessment for Yellow and Green Lights potential admissions
Collaborate with neighborhoods/Communities on determination of Yellow lights
Obtain prior authorization for Managed Care or confirm 3 day QHS for MCR.
Ensure the Long term Care workflow is being followed
Obtain 7000 or complete PAS (Pre-Admission Screen)
Completes sex offender checks on all referrals
Waitlist Referral in CRM. Send Admission notification
Accountable to ensure discharge orders, and appropriate hospital documentation are uploaded into CRM
Accountable to response time for referrals; manage Red/Yellow/Green light responses from each location
Communicate with locations on capabilities, current considerations for admissions, etc.
Ensure communications/notifications are sent to appropriate departments
Verify and update current demographic information
Generate Admissions Agreement prior to Admission, ensure completion (remote) within 72 hours / 3 business days of any new admission or re-admission.
Complete necessary trackers / reports – PASRR, Admission Agreement, etc.
Cross train/provide coverage as needed for various Regions within the organizations.
Responsible for all duties outlined above for assigned locations
Select, train, develop and discipline the other Neighborhood Admissions Specialists
Implement schedules such that weekend coverage is continually provided
Experience & Qualifications
High School Diploma/GED required
Experience with Electronic Medical Records, Customer Relationship Management Software
Expertise in knowledge and understanding of long term care payers
Proficient with: Microsoft Office products
Special Skills and Requirements
Skilled in collaborating with multiple internal and external customers to build and maintain strong relationships
Comfortable with using math skills to analyze data and make recommendations
Skilled in: Conflict Management, Critical Thinking and Customer Service
Ability to prioritize competing priorities
Flexible and adaptable to change
Above Average organization/planning and attention to detail
Proficient literacy, writing, documentations, communication, and interpersonal skills
Recognize and uphold the autonomy and dignity of all elders.
