Director of Rehabilitation

Greene Meadows Nursing is looking for a Director of Rehabilitation (DOR) to lead the IN-HOUSE therapy team! Responsibilities include leading and managing the rehabilitation department within a skilled nursing facility. This includes overseeing physical therapy, occupational therapy, and speech-language pathology services to ensure high-quality, patient-centered care that promotes recovery and enhances quality of life for residents.

The DOR ensures compliance with all regulatory requirements, manages departmental budgets and staffing, and collaborates with interdisciplinary teams to optimize clinical outcomes.

Key Responsibilities

Supervise and support a team of rehabilitation professionals, including PTs, OTs, SLPs, and therapy assistants. Coordinate and monitor the delivery of therapy services in accordance with physician orders, patient care plans, and facility policies.

Develop and implement rehabilitation policies and procedures to ensure compliance with federal, state, and local regulations.

Patient treatment as indicated.

Conduct staff performance evaluations, mentor team members, and facilitate professional development.

Collaborate with nursing, medical staff, and other departments to support interdisciplinary care planning.

Maintain accurate documentation and ensure billing compliance and productivity standards.

Monitor clinical outcomes and quality metrics to drive continuous improvement.

Participate in facility meetings, care conferences, and audits as required.

Maintain current knowledge of industry trends, regulations, and evidence-based practices in rehabilitation.

Qualifications

Graduate of an accredited program in Physical Therapy, Occupational Therapy, or Speech-Language Pathology.

Current state licensure in one of the rehabilitation disciplines (PT, OT, or SLP).

Minimum of 2–3 years of supervisory or management experience in a SNF or post-acute setting preferred.

Demonstrated leadership ability, strong communication skills, and a team-oriented approach.

In-depth understanding of Medicare/Medicaid regulations, MDS processes, and therapy billing/reimbursement models.

Proficient in using therapy documentation software.
