Role and Responsibilities
· Participate in setting goals and developing action plans for the team.
· Lead and supervise our direct care team, ensuring that daily tasks are carried out effectively
· Create an engaging activity calendar for ADS
· Review ISP's, attend ISP meetings, share information with team
· Build relationships with community stakeholders and promote our ADS program
· Ensures the needs, interests and choice of persons served are met across environments, both agency controlled and in the community
· Ensure quality assurance and improvement functions within the Day Programs are developed and implemented.
· Identifying trends as it relates to program performance measurement, key performance indicators, and outcomes
· Reviews and tracks UIR's
· Serves as the program point person as it relates to audited files or any other form of governing agency audits
· Complete QA file reviews to identify file compliance levels and track action plans to correct file deficiencies
· Responsible for all safety activities in the workplace
· Document all accidents and safety hazards and authorize all needed repairs/corrections
· Collaborates with program leaders and managers to ensure staff follow through on all documentation
· Conducts quarterly environmental reviews and follows up with program leadership in addressing plans of correction
Qualifications and Education Requirements
· High school diploma
· Clean background check
· Valid driver’s License with 6 or less points
· Valid car insurance (liability required)
Preferred Skills
· Applicant should also have a background serving individuals diagnosed with developmental disabilities.
· Must want to care for and provide caring services to our consumers
