WHO WE ARE LOOKING FOR
The Coordination of Community Services Program, within Aging and Disability Services, is currently recruiting for an enthusiastic, patient, detail-oriented professional who will serve as a Coordinator that is dedicated to assisting participants with intellectual and developmental disabilities and access the resources and services they need to live successfully in their communities.

The Coordinator of Community Services (CCS) plays a vital role in advancing the organization’s mission by ensuring individuals receive comprehensive, person-centered coordination of services. This position supports individuals with complex needs by connecting them to appropriate resources, monitoring service delivery, and promoting stability, independence, and community integration. The CCS serves as a primary liaison between individuals, families, service providers, and internal stakeholders to ensure quality outcomes and regulatory compliance.

Core Duties and Responsibilities

Develop, implement, and monitor individualized Person-Cantered Plans in collaboration with the individual, family members, and service providers.

Coordinate and link individuals to community-based services, benefits, and supports.

Maintain regular contact with individuals to monitor progress, ensure service effectiveness, and address emerging needs.

Conduct required home and community visits in accordance with program standards and timelines.

Monitor provider performance and ensure services are delivered in compliance with applicable policies, regulations, and contractual requirements.

Maintain accurate, timely, and thorough documentation in electronic record systems (LTSS, Long Term Services and Support).

Track billing activities and ensure required documentation supports program and funding requirements.

Collaborate with interdisciplinary teams, community partners, and stakeholders to ensure coordinated care.

Experience
Two (2) Years of professional

Case Management experience , providing services to participants with IDD (intellectual and/or developmental disabilities) who are enrolled in Home and Community-Based Waiver services.

Education
Graduation from an accredited college or university with a Bachelor's Degree in a human services field (Human Services, Social Work, Psychology, Sociology, Public Health, Public Administration, Human Development, Family Studies, Family Science, Counseling, Behavioral Science, Community Health, etc).

Equivalency
An equivalent combination of education and experience may be substituted. (Note: For positions in the DHHS’ Coordination of Community Services Program, education and experience, requirements may be waived if an individual has been employed as a coordinator of community services for at least 1 year).

License
Possession and maintenance at all times of a valid Class "C" (or equivalent) driver’s license from the applicant's state of residence when required for job related duties.

At least 2 years of experience in Case Management services for participants with IDD who are enrolled in Home and Community Based Waiver services.

Experience with Medicaid-funded programs, waiver services, or state-regulated service coordination models.

Experience collaborating with a variety of provider agencies, MD Dept of Health and stakeholder groups.
