Responsibilities

Following a pre-set care plan
Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
Assist with medication reminders
Monitor and report on client's well-being
Light housekeeping, laundry, and other household daily chores as needed
Grocery shopping and errands
Cooking/meal preparation
Scheduling appointments, including doctor's
Communication with Family Ties HC management and family members/loved-ones regarding the client's daily status
Daily notes
Other duties assigned as needed
Qualifications

Current HCA or higher certification
Caregiving experience of 1 year or similar experience
Compassionate and caring demeanor
Excellent communication & interpersonal skills
Ability to follow the pre-set care plan and medication schedule
Ability to lift 35 pounds
Ability to assist with client's mobility as needed
Valid driver's license and reliable transportation
Ability to work the schedule as described
