Qualifications

Registered Nurse (RN) license with a minimum of 3 years of healthcare experience, preferably in hospice or related settings

Bachelor's degree strongly preferred

Leadership or management experience a major plus

Valid driver's license (required)

Responsibilities

Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more

Ensure adherence to hospice regulations and standards

Facilitate communication between medical staff and the care team

Lead interdisciplinary meetings, documenting patient needs and care plans

Develop and update patient care policies and emergency procedures

Manage staff scheduling, performance reviews, and foster a positive work environment

Act as the central hub for communication with families, physicians, and other providers

Analyze quality metrics and manage a caseload of patients as needed

Participate in after-hours and on-call duties to ensure seamless care

Maximize resource utilization and promote a collaborative work culture
