Job Description UAS Assessment Nurse (RN)

Compensation Salary Range: $90,000 - $108,000 per year + $15,000 Sign-On Bonus

Location: Various Territories, NYC Field-Based / Onsite

Position Summary

As a UAS Assessment Nurse (RN), you will play a vital role in evaluating the clinical, psychological, and social needs of members in the community. This field-based position involves conducting in-home and nursing home assessments using the UAS-NY system, ensuring compliance with New York State Department of Health regulations. You will contribute to developing person-centered care plans and supporting improved patient outcomes.

Responsibilities

Shift details: Monday - Friday (Business Hours)

Hours per week: 40 ? Key Duties:

Conduct initial and reassessment visits, determining eligibility and care needs using UAS-NY

Perform functional, psychosocial, and environmental safety assessments

Develop and communicate individualized, person-centered service plans

Educate members and caregivers on program benefits, services, and guidelines

Submit accurate, timely documentation and communicate urgent needs to care teams Requirements

? Education

Required: Associate's Degree in Nursing

Preferred (if applicable): Bachelor's Degree ? Licenses & Certifications

Required: Active Registered Nurse (RN) license; UAS-NY Certification ? Skills:

Strong knowledge of community health and managed care practices

Excellent assessment and clinical evaluation skills

Proficiency in Microsoft Word and Excel

Strong communication and organizational skills
