Oak Street Health is a primary care organization focused on older adults on Medicare. The Community Relationship Manager (CRM) leads outreach and community engagement efforts to generate new patient growth.

Pay Range: $21.10 - $40.90 (hourly)
Key Responsibilities Community Outreach / Engagement
Research local community influencers and partners in the assigned market.
Prospect and cold‑call local businesses such as senior buildings, libraries, churches, and senior groups.
Leverage community relationships to impact patient acquisition and lead generation targets.
Identify opportunities to expand and develop new patient growth channels.
Collaborate with the Regional Partnership Manager to build relationships with insurance agents.
Account Management
Build and nurture long‑term alliances with local leaders (field account managers).
Act as the point of contact for both the Outreach Team and community partners.
Maintain regular touch‑points and discuss KPI reviews, status updates, and new initiatives.
Oversee cross‑collaboration initiatives and evaluate success through lead generation and patient growth metrics.
Event Planning
Develop and manage a 6‑plus‑week calendar of community events.
Coordinate event details to ensure engagement, attendance, and lead generation.
Communicate objectives, strategy, and expectations with relevant stakeholders.
Sales
Generate field account engagement through proactive outreach to local leaders.
Maintain knowledge of internal sales systems and marketing programs.
Plan lead generation and community engagement activities at least two weeks in advance.
Perform additional duties as assigned.
Qualifications Previous experience in account or community engagement.
Comfort with business‑to‑business cold calling and prospecting.
Proficiency in daily accountability on key metrics (touch‑points, events, leads, patients).
Ability to adapt and overcome external relationship challenges.
Strong multi‑project management skills.
Proactive mindset for generating new growth opportunities.
Public speaking skills and strong written and verbal communication.
Bachelor’s degree or equivalent work experience.
Fluency in Spanish, Polish, Russian, or other relevant languages (where necessary).
U.S. work authorization.
