Position Overview Position Type: Contractual (8/30/2026 – 8/29/2027)
Schedule: Full-time – Shift as needed – 8:00am-4:30pm; 35 hours per week – Saturday, Sunday, Monday, Tuesday.
Salary: $60,000 per year.
Summary This position manages, coordinates, and participates in the development of community resources in partnership with both public and private agencies within the assigned community. The role involves partnering with existing coalitions, establishing new partnerships to plan programs, services, and supports tailored to families with newborns, and collaborating on marketing and public relations efforts for Family Connects Maryland. The position also maintains communications among community resources, physicians, nursing personnel, and other health professionals regarding program objectives, policies, and procedures, while compiling and maintaining records.
Responsibilities Nurture relationships with community resources and services that support families referred by Family Connects nurses, ensuring timely and appropriate engagement within the community system of care for young children and their families.
Identify and establish relationships with providers, maintain an updated resource list, and identify gaps to help the community grow necessary resources over time.
Provide individual-case consultation on community resources to support nurse home‑visitors when connecting families with resources.
Market the program by communicating with referral sources (ob/gyn practitioners, hospitals, pediatricians) and groups interacting with families (employers, newspapers, churches, child care agencies, the public). Solicit support from community leaders to publicize the program and provide incentives for families.
Increase options for pregnant women and new mothers to learn about and sign up for Family Connects home visits by collaborating with health providers and other organizations.
Establish a community advisory board for the Family Connects program to coordinate services with other early childhood system of care stakeholders.
Assist in initial contact with families to successfully engage, recruit, and schedule home visits.
Organize and participate in enrollment scheduling at Sinai Hospital for mothers who have given birth.
Education Bachelor’s Degree from an accredited college or university in Social Work, Human Services, or Public Health.
Experience Six (6) years of progressive experience in early childhood/prevention services; home visitation experience preferred.
Knowledge of infant/child development, parent-child attachment, maternal-infant health, and dynamics of child abuse and neglect.
Experience working in community-based programs preferred.
Experience and willingness to work with a culturally diverse population.
Skills Knowledge of community resources and agencies.
Knowledge of Jewish customs, practices, and traditions.
Knowledge of infant and child development.
Ability to build trusting relationships and communicate clearly and professionally with a range of community stakeholders.
Excellent oral and written communication skills.
Ability to work independently and as part of a team.
Work Environment Must be prepared to travel to various program sites and communities.
A driver’s license and insured vehicle are required to fulfill the position’s duties.
