Care Coordination & Community Relations Coordinator

to join our growing team.

This is more than an office job. You'll build meaningful relationships with seniors, families, caregivers, referral partners, and the community while helping ensure every client receives exceptional care.

What You'll Do

As an important member of our leadership team, you will:
Coordinate care between clients and caregivers to ensure an exceptional client experience.
Visit clients throughout Corona, Norco, Eastvale, Chino, Chino Hills, and Diamond Bar.
Build trusting relationships with seniors, families, and caregivers.
Work closely with our Scheduler and Recruiter to find the best caregiver-client match.
Support caregivers through coaching, communication, and problem-solving.
Assist with client assessments, care plan updates, and quality assurance visits.
Help maintain accurate documentation and office records.
Learn the day-to-day operations of the business and make sound decisions when situations arise.
Assist with office administration and special projects.
Represent Right at Home at community events, networking meetings, and marketing activities.
Build relationships with healthcare professionals and community partners.
Respond calmly and professionally to unexpected situations while maintaining excellent customer service.

Qualifications
High school diploma or equivalent.
2+ years of experience in healthcare, customer service, office administration, staffing, recruiting, hospitality, or business operations.
Previous home care, healthcare, senior care, or caregiving experience is preferred.
Strong organizational and computer skills.
Excellent verbal and written communication.
Valid driver's license, reliable vehicle, and willingness to drive throughout our service area.
Ability to lift up to 25 pounds occasionally.
Bilingual (English/Spanish) is a plus.
