Communications Support For Population Health Management

Support strategic communications for Population Health Management (PHM), Managed Care Plan (MCP), and Behavioral Health Plan (BHP) initiatives.
Develop and edit communication materials, including presentations, project updates, newsletters, and stakeholder communications.
Coordinate stakeholder reviews, track feedback, and manage approvals to ensure timely delivery of communications.
Maintain communication plans, project schedules, and deliverable trackers to support project execution.
Partner with business, clinical, operations, and project teams to ensure clear and consistent messaging.
Prepare executive summaries, status reports, and meeting materials for leadership and stakeholder meetings.
Support change management and stakeholder engagement efforts related to healthcare program implementations and enhancements.
