Program Manager, Pharmacy Operations (PGY1 and PGY2 required)
Program Manager, Pharmacy Operations is a Clinical Pharmacist responsible for comprehensive medication management of patients with Non‑Tuberculous Mycobacteria, Bronchiectasis, and Cystic Fibrosis within the Division of Pulmonary, Critical Care and Sleep Medicine outpatient practice. This part‑time role (17.5 hours) focuses on optimizing therapeutic outcomes, minimizing drug‑related problems, and ensuring safe use of complex medication regimens.
Job Responsibilities
Direct Patient Care Responsibilities
Coordinate all pharmacotherapy needs in collaboration with physicians, clinical team members and program directors.
Maintain contact with assigned caseload between visits, initiating responses to issues and problems.
Communicate with the multidisciplinary team to optimize patient care, interfacing with physicians, social workers, nurses, nurse practitioners, dietitians and others to plan and execute specialized protocols.
Inform physicians of patient status and needs, monitoring clinical indicators to coordinate care with the patient’s physician.
Provide patient and family education, ensuring excellent patient services and satisfaction.
Develop and maintain a high‑level clinical pharmacy practice, participate in multidisciplinary bedside rounds, and provide pharmacotherapeutic, pharmacokinetic and pharmaco‑economic consultation.
Counsel patients and caregivers on medication use and participate in educational initiatives.
Develop and implement clinical practice guidelines that enhance pharmaceutical care and medication safety.
Document and report clinical pharmacy interventions, adverse drug reactions, and medication errors.
Education, Development and Research
Promote own professional growth in clinical and managerial roles.
Serve as a clinical expert and resource for the education of peers and other health professionals.
Participate in educational and outreach programs for patients, families, and health professionals.
Assist in preparation, implementation, and evaluation of research protocols when applicable.
Lead investigator‑initiated trials, advancing the team’s research productivity.
Minimum Qualifications
Doctor of Pharmacy degree from an ACPE accredited school.
Active New York State Pharmacist Licensure.
PGY1 pharmacy residency training.
PGY2 pharmacy residency training or equivalent salaried professorship at a pharmacy school.
Board certification in Pharmacotherapy or another specialty required within 18 months of hire.
At least four years of pharmacy experience.
Effective leadership skills; self‑directed; able to work independently.
Excellent interpersonal, communication and organizational skills.
Creative problem solving and critical thinking ability.
Understanding of complex clinical care protocols and familiarity with state and federal medication regulations.
Preferred Qualifications
