The ACN and DCPH consists of 14 primary care sites, seven school‑based health centers, 13 mental health school‑based programs, and over 60 specialty practices. It also oversees more than 20 community programs, including a NYS Health Home Program. The Comprehensive Health Program (CHP), the site of this position, is a center of excellence for HIV treatment and prevention as well as sexually transmitted infections and general infectious diseases. CHP embraces comprehensive care coordination strategies and multidisciplinary, team‑based care, including integrated behavioral health services, intensive care coordination, and primary care. Patients are cared for by a team of multidisciplinary providers (care coordinators, social workers, registered nurses and primary care providers) and are offered Health Home enrollment as part of this comprehensive care.

Program Manager – Comprehensive Health/Health Home & Care Coordination – Grant Funded
The Program Manager will oversee care coordination programming and Health Home program activities. Responsibilities include leading the care coordination and Health Home programs alongside other Program Managers, supervising day‑to‑day operations, monitoring billing, documentation, and data reports, and ensuring compliance with program requirements. The Program Manager will guide implementation of the strategic vision, supervise care coordinators, provide coaching and feedback, and interpret data reports to maintain high fidelity of data. Experience with grant‑funded program implementation, evaluation, and management will be required. The Program Manager will participate in Care Coordination Steering Committee meetings, build relationships with NYP departments and community organizations, and report program outcomes to funders, senior leadership, and stakeholders.

Preferred Criteria

Bilingual English/Spanish

Master’s in social work or related clinical field, or Master’s in Public Health

Experience in program management, development, implementation and evaluation

Interest in quality improvement strategies/planning

Familiarity with care coordination and program management including ensuring deliverables are met and reporting to key stakeholders

Comfort with data reports and tasking team members to achieve high quality outcomes for patients and meet programmatic requirements

Required Criteria

Bachelor’s Degree

Minimum of 5 years of experience

Minimum of 2 years supervisory experience

Strong written and oral communication skills

Experience in program development, implementation and evaluation

All roles require on‑site presence; therefore, employees should live within a commutable distance to NYP. NYP will not reimburse travel expenses.
