Reentry & Community Support Coordinator

Weber County is seeking a dedicated Reentry Case Manager to support individuals transitioning from incarceration back into the community. You will assess needs, create individualized reentry plans, coordinate access to housing, healthcare, and treatment services, and advocate for resources to reduce recidivism.
This role requires collaboration with correctional and community partners, adherence to policy, and ability to navigate complex systems to help clients achieve successful reintegration.
