PRP Coordinator

Overview

Treasure Health Systems seeks a PRP coordinator to implement the rehabilitation activities outlined in the client's Individual Treatment Plan (IRP). The Rehabilitation Specialist supervises this position and provides direct client services, including intake assessments, individual/family/rehabilitation group treatment and crisis intervention.
Responsibilities

Assess the needs of clients throughout their time in the program.
Provide services weekly in the home, office, community or remotely.
Complete all notes (direct and indirect) on EMR daily.
Support the Rehabilitation Coordinator in updating treatment plans in EMR for each client, detailing progress toward goals, response to services, and a follow‑up plan.
Attend weekly team meetings and training as scheduled.
Participate in monthly supervision with the Rehabilitation Specialist.
Collaborate with the client’s assigned mental health therapists monthly and document the type of collaboration (e.g., phone call, email, attending a meeting).
Perform other duties as assigned.
Additional Requirements

Daily and/or weekly home visits to clients’ homes (currently telehealth services).
Use own vehicle for visits to clients’ homes.
Valid driver’s license.
Must have experience working with children and adults in PRP.
Naloxone training.
CPR training.
DLA-20.
Qualifications

Bachelor's degree in a health‑related field.
Associate degree in a health‑related field.
30 hours of college credit toward a bachelor's degree in a health‑related field, or one year of work experience in a supervised mental health setting or PRP experience is preferred.
