What You'll Do

Coordinate client care services, including assessments, care planning, client visits, and caregiver introductions

Supervise and support caregivers through coaching, orientation, quality assurance visits, and performance management

Serve as a primary point of contact for clients, families, caregivers, and referral partners

Collaborate with the Staffing Coordinator to ensure appropriate caregiver-client matches

Maintain accurate documentation and support compliance requirements, including long-term care insurance documentation

Participate in an on-call rotation to address urgent client and caregiver needs

Work collaboratively across departments and perform other duties as assigned to support the success of our clients and team

Requirements

Minimum of 2 years of experience in a care-related field

Previous supervisory or leadership experience

Strong communication, relationship-building, and problem-solving skills

Ability to lead, coach, and hold team members accountable

Valid driver's license, proof of insurance, and ability to travel locally

Basic computer proficiency, including Microsoft Office and data entry

Ability to successfully pass a criminal background check

Schedule

Full-time position

Monday–Friday, 8:30 a.m. – 5:00 p.m.

Participation in a shared evening and weekend on-call rotation

Some flexibility required based on client needs

Compensation
